review request: 4975 missing mutex_destroy() calls in zfs

Note: we are bringing this change from OpenZFSonOSX to minimize code
differences with them. The *_destroy() calls are more critical there,
because they actually free memory. The primary enhancement for illumos is
just the added assertions in the *_destroy() routines.
